68Ga-fibroblast activation protein inhibitor PET/CT on gross tumour volume delineation for radiotherapy planning of oesophageal cancer

Background and purposeTo compare 68Ga-fibroblast activation protein inhibitor (FAPI) 18F-FDG PET/CT in imaging locally advanced oesophageal cancer, evaluate the potential usefulness of 68Ga-FAPI on gross target volume (GTV) delineation aimed at radiotherapy planning for cancer as compared with contrast-enhanced CT (CE-CT) PET/CT.Materials methodsTwenty-one patients newly diagnosed who underwent both scans were selected. GTVs primary tumours based CE-CT (GTVCT), PET/CT, plus delineated. Gross tumour lengths measured by endoscopy recorded.ResultsThe PET showed significantly higher radiotracer uptake than (median SUVmax 16.71 vs. 11.23; P = 0.002) tumours. SUV thresholds FAPI ×20%, 30%, 40%, FDG ×40% similar lesion that endoscopic examination (P > 0.05). GTVCT demonstrated largest (median: 48.80 mm3, range: 14.83–162.23 mm3) PET-based GTVs. For PET/CT-guided complementary contouring GTVCT, four (19%) increased ×20% two (9.5%) ×40%, only one patient was ×40%. Furthermore, GTV 40% no significant difference FAPI-PET meets organ risk standard.ConclusionThe methodology favourable tumour-to-background contrast might provide additional information help avoid geographic misses.
